  • Clients must be Medi-Cal eligible

Overview

Certain supported housing placements also include case management services. In Solano County, these services are provided at Laurel Creek, Gateway, Laurel Gardens, Sereno Village, Blue Oak Landing, Fair Haven Commons, Rocky Hill and MHSA Permanent Housing. In San Mateo County, they are provided at Colma Ridge, Tahanan, and community-based housing units.

Multiple levels of case management services are available, and clients are placed at the most appropriate level depending on their needs. Case management services include assessments, individualized treatment plans, integrated health services, housing assistance, crisis intervention, and case management. Many case management programs also offer psychiatric and medication management services.
