    • Assessed through the County Access Line
    • Over the age of 18
  • For Solano County services, clients must be:
    • Residents of Solano County
    • Clients of Solano County Behavioral Services
    • Medi-Cal eligible

Overview

Case management services coordinate community-based services by a professional or team to provide quality mental health care customized accordingly to individual clients' needs and aid them in their recovery. Multiple levels of case management services are available, and clients are placed in the most appropriate level depending on their needs. 

Case management services include assessments, individualized treatment plans, integrated health services, housing assistance, crisis intervention, case management, and forensic advocacy. Most case management programs also offer psychiatric and medication management services.
